Military Identification Cards & CAC Cards

Active Duty:
- Is your ID card damaged?
- Is your ID card expired?
- Have you advanced or been promoted?
- Have you re-enlisted or have an extension due to begin?
- Did you lose your card?
If you answered YES to any of those questions, you need a new Active Duty ID card.
For a Lost/Stolen ID Card, a memo will need to be completed and routed. Click here for a sample memo.
Dependent ID Card:
Dependents and dependents of Retirees must have the sponsor present at the time the card is being issued.
If you:
- Are a newlywed
- Have a new born child
- Have adopted a child
- Have a dependent over 21 and under the age of 23 (and still in college)
If you answered YES to any of those prompts, you need a new Active Duty ID card. You will need to provide proper documentation: A marriage certificate, birth certificate, adoption papers, or a letter from the school stating the student is in school full tijme including the anticipated date of graduation.
Types of Identification Required:
TWO (2) forms of Identification are required for CAC and Military ID Cards (including at least one (1) photo ID). Please see the attached document for the appropriate and acceptable formsof military identification documents.

a. Open the document.
b. Complete Blocks 1a. 1b. 1d. 1e. 1f. 1g. 1h. 1i. (1c. should be filled out if they don't have an SSN).
c. Have a 6 to 8 digit (numerical) Personnel Identification Number (PIN) in mind as you will need to provide this when they are processing your action at the CAC Issuance Office.

Active Duty andReserve Members
Two forms of identification; one must have a picture, bring your current/expired government issued ID card and your work e-mail address. Be sure to print clearly your full internet e-mail address (not your display name). (i.e. Im.a.Coastie@uscg.mil)
* If you don't have a work e-mail account, you must obtain one from your parent command before a CAC card will be issued.
* If you bring the wrong e-mail address you will have to return later to correct it.
* Personal e-mail addresses (i.e., AOL accounts, etc., ) will not be accepted.

Retirees
Bring your current/expired government issued ID card and your retirement orders or DD Form 214.

Dependents
To add dependents
*Dependents must have a DD-1172 signed by the sponsor and notarized or the sponsor must be present at the time the card is issued. The sponsor brings in the marriage certificate or birth certificate/certificate of live birth. A Social Security Card or other official Government document reflecting the SSN such as passport, driver's license, and letters from the SSA is required when verifying the SSN for enrolling or updating members or dependents in DEERS.
Documentation to bring - a marriage license or certificate; birth certificate; drivers' license; social security card; passport; voters registration card; school ID; adoption papers; a letter from the school registrar certifying full-time course of study leading to an associate's degree or higher and anticipated graduation date.
NOTE: A valid power of attorney is acceptable when the sponsor is not present to sign for a family member's DEERS enrollment.
If already enrolled/renewing
Dependents must have a DD-1172 signed by the sponsor and notarized or the sponsor must be present at the time the card is issued.Bring your current dependent ID Card. If your ID card is expired, you must also bring a current picture ID issued by the state or government as well as one form of additional documentation (please see list below). If the sponsor is unable to accompany their dependents due to long term deployment (greater than 90 days) or if you have any questions or concerns prior to obtaining an ID Card, please contact the customer service desk at (504) 253-4791.
Authorized additional documentation to bring - a marriage license or certificate; birth certificate; drivers' license; social security card; passport; voters registration card; school ID; adoption papers; a letter from the school registrar certifying full-time course of study leading to an associate's degree or higher and anticipated graduation date.

Civilian Employees
NAF/GS/WG civilian employee's service records are sent to DEERS via an authorized data feed. We can no longer enter any information into DEERS for civilian employees.
Civilian Employees must have their Civilian Personnel Office enter their information into DEERS. If you are not in or updated in DEERS we cannot process your CAC.
Civilian Employees that have enrolled and are entered into DEERS should still call to verify DEERS enrollment at 1-800-538-9552 - Option 3 and ensure your end date is (unknown).
NOTE: Newly hired Civilian Employees should anticipate a two week wait for DEERS enrollment.

Contractor Employees
DoD's Contractor Verification System (CVS) is the only way that a contractor employee can obtain a CAC. Only contractors who have access to classified materials or access to Federal IT systems (with or without unescorted access to our facilities) will be inputted into CVS.
Contractor employees that have enrolled and are entered into DEERS should still call to verify DEERS enrollment at 1-800-538-9552 - Option 3.

Reminders:
- Sponsors must accompany their dependents for their dependent to receive an ID card (unless the sponsor is deceased).
- Newly-married spouses must bring their marriage certificate to update dependency data and enroll in DEERS.
- If a child over 21 requests an ID card, the child will need to bring a letter from the registrar of his/her college to show that he/she is a full-time student.
- Children under the age of 10 do not need dependent ID cards.
