Hours of Operation:

Monday - Thursday: 0700 - 1530

Friday: 0700 - 1130*

*(Please note that the clinic is closed for appointments on Friday's beginning at 1130 for Training and Medical Administration time)

Duty HS onboard M-F: 1600-1800

The Clinic is closed daily from 1130-1230 for lunch.

DENTAL SERVICES

We provide all routine dental care, including preventative, restorative, surgical, and prosthetics to Active Duty personnel only.  Dental Hygiene services are also available.

DENTAL CLASSES

Class 1: Have a current dental exam & does not require treatment or reevaluation. Worldwide deployable.  

 

Class 2: Have a current dental exam and require non-urgent treatment or reevaluation for oral conditions, which are unlikely to result in dental emergencies within 12 months. Class 2 are worldwide deployable.

 

Class 3: Patients who require urgent or emergent dental treatment. Class 3 patients normally are not considered to be worldwide deployable.

 

Class 4: Patients who require periodic dental examinations or patients with unknown dental classifications. Class 4 patients normally are not considered to be worldwide deployable

 

DENTAL READINESS

When an individual goes to a civilian dentist (Active Duty Dental Plan) or DOD dentist for an exam or treatment, the DD-2813 shall be filled out and sent via fax to (504) 253-4717 or scanned and emailed to:  D08-SMB-BaseNOLA-HSWL-Clinic@uscg.mil. Click here for the DD2813 Dental exam form.
