Notice of Eligibility (NOE) Process

BACKGROUND

A NOE is used to authorize medical and/or dental treatment for a reservist who incurs or aggravates an injury, illness or disease in the line of duty (active or inactive duty).NOEs are only issued to reservists NOT currently on active duty orders. Medical and/or dental treatment authorized under a NOE is limited to treatment specific to the line of duty injury, illness or disease and is typically only issued for a specific time-period. Chapter 6 of the Reserve Policy Manual Reserve Policy Manual, COMDTINST M1001.28 (series), provides additional information regarding NOEs.

Additional benefits that may be authorized under a NOE include:

  • Reimbursement for travel and transportation connected with the medical and/or dental treatment.
  • Readiness Management Period (RMP's).
  • Incapacitation pay.

REQUEST PROCESS

All NOE requests must include the items below and be routed through the command and District RFRS (or equivalent) to CG PSC-RPM-3 via email to: ARL-DG-CGPSC-RPM-Reserve-Medical@uscg.mil.  All personnel involved in this process shall ensure proper precautions one taken to protect Personally Identifiable Information (PII) and Protected Health Information (PHI) especially when transmitting electronically.

Initial requests must include:

  • Signed Reserve Incapacitation Benefit Request Form.
  • A signed Injury Report (CG-3822), Report of Illness of Reservist (CG-4614) or a letter of incident report (LIR) documenting the line of duty (LOD) injury, illness or disease.  An appropriate line of duty (LOD) determination shall be completed IAW Chapter 7 of the Administrative Investigations Manual COMDTINST M5830.1(series).
    • Note: A formal LOD investigation is only required when the command has reason to believe that the injury, illness or disease was not incurred in the line of duty or occurred as a result of misconduct or gross negligence.
  • All relevant medical documentation from Coast Guard, civilian, or non-CG military treatment facilities.
  • A current Monthly Physician's Report Form.
  • Signed Authorization for Disclosure of Medical or Dental Information, DD Form 2870.
  • Signed copy of the active duty orders or Direct Access screenshot of the members IDT, FHD or RMP.
  • A signed administrative remarks, CG-3307, (RIB-01) (NEW REQUIREMENT)
  • A signed administrative Remarks, CG-3307, (RIB-04) if you are transitioning from either Medhold or ADHC orders.

CG PSC-RPM-3 will document the NOE in the Medical Readiness Reporting System (MRRS). This action will change the member’s medical Deployment Limiting Conditions (DLC)  in CGBI to red, to ensure members are not mobilized, involuntarily or voluntarily, until Fit for Full Duty (FFD) in accordance with chapter 3 of the Reserve Policy Manual, COMDTINST M1001.28 (series).

ADT-AT WAIVER

As a reminder, Commands need to ensure waivers for Active Duty for Training - Annual Training (ADT-AT)  for purpose of a disability are authorized as needed, in accordance with chapter 4.B.3 of the Reserve Policy Manual, COMDTINST M1001.28 (series).
